You will need Adobe Acrobat Reader to display PDF documents. Acrobat Reader is available for free at http://www.adobe.com.
The PDF document will open in your browser window. To fill out a form, click on the hand icon and then in the first form field. A cursor will appear in the first form field. Type directly into the PDF and use the tab key to go from one field to the next. For check box fields, the cursor will turn into a pointer finger. Click to place a check in the box.
You can print the document and/or save it to your desktop.
